At the time of the study, the hospital had 4,000 discharges a year and 10 individual programs (cardiopulmonary, stroke/neurological, transitional care unit, musculoskeletal, spinal cord/addictions, brain injury, complex medical/oncology, amputee, pain, pediatrics). Always Events refer to aspects of the patient experience that are so important to patients and families that health care providers should reliably implement the events 100 percent of the time.The study site was Spaulding Rehabilitation Hospital (Boston), a 296-bed state-licensed rehabilitation hospital. This content was initially created as part of the Picker Institute’s Always Events Program. The Planetree Same Page Patient Notebook includes detailed information and tools that are designed to be useful to patients, care partners, and the health care team. The tools include surveys to fill out before and after a patient’s stay as well as specific resources designed to support care partners. The goal is to support patients, their care partners, and the team of health care providers to all be “on the same page” in understanding the patient’s health and health care needs when the patient is transitioning from one setting of care to another. These resources and tools were developed for patients and their caregivers or care partners to use when planning for care or during a stay in a hospital or skilled nursing facility.
